Not sure what the hotfix patch did, but last week I was atleast getting 40-50 FPS, now I am getting any where between 19 and 27. My eyes are bleeding and this is pretty much unplayable.

I am also having the exact same issue. FPS have gone from 50+ to 20 with stuttering every second. I have tried all of the presets but nothing helps. Interestingly my CPU is getting maxed out on the performance monitor and the render graph on the gpu graph spikes to 100%.
For info I am on Linux
AMD Ryzen 9 3950X 16-Core
Driver: X.Org AMD Radeon RX 5700 XT (NAVI10, DRM 3.39.0, 5.9.1-1-default, LLVM 10.0.1)
Driver Version: 4.6 (Compatibility Profile) Mesa 20.2.1
OpenGL Version: 4.6 -
Do you have a Hammer Transferking in your inventory?
This machine is bugged, it makes a second sun appear, thus killing the performance. Sell it and everything will be good.
Interestingly I had similar performance issues even without the bugged trailer, but after some testing in a different savegame (buying and selling the Transferking) the issues kinda disappeared, even in my normal savegame.Die Kunst der Programmierung besteht darin, eine leere Textdatei mit Bugs zu befüllen

This image shows the performance spikes which go away if I look backward out of the Cab or in 3rd person -
Please select a different Graphics Preset, save and Chose the one you like, save.

nope tried low through to ultra. was fine on ultra before this last update! Something changed with this patch. If you read my first post in this thread I tell you I have tried different settings so this isnt the issue.

You never answered If you own Transfer King Trailer.
PS:
You can try disabls Antialiasing Settings in your Driver die CNC because CNC seems to use own AA Mode automatically.
Make Sure you disable AA in your Graphics Settings. "Let application Decide" is Not enough."
PPS:
For me First Run it was slow. Walking around a bit solved the issue. Perhaps shader cache.
Check your Video memory. 4.5 -5GB in Texture Ultra and 1920x1080. If you use Higher Resolution this May increase above 6GB. -
I realised the same issue yesterday as I played the 4th scenario.

I just found an interesting fix after my fps slowly diminished to about 15 over the past hours of intense field work... save the game into a new slot and your fps might jump up again.
Although my performance issues seem to be different to yours, i don't have those spikes, this may work for you too.Die Kunst der Programmierung besteht darin, eine leere Textdatei mit Bugs zu befüllen

The scenario does provide the transfer king and there is indeed two suns set about 15 to 20 degrees apart so your diagnosis may well be correct.
